By checking this box and submitting this registration form, I acknowledge that I am the parent/legal guardian of the child being registered. I give permission for my child to travel with First Baptist Church of SW Broward to attend Junior Camp at Camp Glory in Haines City, Florida, for the week of July 13–17, 2026. I understand that this trip includes transportation to and from camp, participation in camp activities throughout the week, and attendance at the planned Legoland outing on Friday. I further give permission for my child to be supervised by approved church leaders, workers, and chaperones during this trip. In the event of an illness, accident, or emergency, I authorize the church leaders/chaperones to obtain necessary medical treatment for my child if I cannot be reached immediately. By submitting this form, I confirm that I have read the trip information, understand the nature of the trip, and give my full consent for my child to attend Camp Glory and participate in the Friday Legoland trip.
